Our goal at Riverside Eye and Laser Specialists is to make your visit as pleasant and hassle-free as possible. Prior to your appointment, we encourage you to fill out the new patient paperwork that is located on this website so that you can accelerate your check-in process. Please download, print and complete the paperwork and bring it with you to your appointment. If you are unable to download our patient paperwork, then it is recommended that you arrive to your appointment between 15-30 minutes early so that you can have ample time to fill out the appropriate forms. Please make sure you also have your driver’s license and insurance cards with you at your appointment so we can make a copy of them. We will also need a list of all the medications you are taking for your chart.
